Email: Keep it professional and you must not use your current work email here to prevent unnecessary problems. You should use an email address that includes your name so that the recipient knows who is sending the message and to covey the tone of professionalism.
Address and Telephone number: The best is to add your city and the state. A telephone number is a must because it is the most common way of contacting a candidate and it must be the most accessible one too.
Other Details: Your LinkedIn profile and the web is a choice if you already have any of those you can add them to.
